All mental health therapy services including Pain Reprocessing Therapy (PRT) therapy intensives and EMDR therapy intensives are provided through Restward: A Marriage and Family Therapy Professional Corporation (dba “Chronic Pain Intensives”, dba “Chronic Pain Therapy Intensives”) a Professional Corporation established in California, United States. Therapy services are provided by Christine Archer, LMFT, LPCC. Christine Archer currently holds the following licenses to provide mental health therapy services in the states of California and Colorado: California Licensed Marriage and Family Therapist (LMFT) #137553; California Licensed Professional Clinical Counselor (LPCC) #15645; Colorado Licensed Marriage and Family Therapist (LMFT) #2448.
All coaching services services including Pain Reprocessing Therapy (PRT) coaching intensives and other non-professional services are provided through Restward LLC dba “Chronic Pain Coaching Intensives”, a Limited Liability Corporation established in New York, United States. Coaching, coaching intensives and non-professional services (”these services”) and products provided by Christine Archer are not offered as a substitute for mental health care or mental health services. When providing these services and products and when working in a coaching capacity through Restward LLC, Christine Archer is not acting as a mental health practitioner and does not purport to offer mental health care. If Christine Archer determines that mental health care services may be helpful, appropriate referrals will be provided.

No Surprises Act & Good Faith Estimate (GFE) for Therapy Clients of Chronic Pain Therapy Intensives
If you receive mental health therapy services including therapy intensives, you have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under Section 2799B-6 of the Public Health Service Act, health care providers and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.
Under the No Surprises Act, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
